AIM is a palliative
Care Program administered through Home Health. The patient may
continue to seek the benefits of curative treatment and
rehabilitation. This allows the family the time necessary to
research options associated with the illness.
The AIM program helps patients, who are currently under the care of home health for serious illness, cope and adjust to possible end-of-life care.
During this time, the patient may continue to seek the benefits of curative treatment and rehabilitation. This allows the patient and family the time necessary to research options associated with the illness.
The AIM program offers palliative care services to
assist the patient and their loved ones, should it become
necessary, with the progression from curative to comfort care. The
AIM Team can transition with the patient to Concerned Care Hospice
should they require this pathway of care.
