Advanced Illness Management (AIM)


AIM is a palliative Care Program administered through Home Health. The patient may continue to seek the benefits of curative treatment and rehabilitation. This allows the family the time necessary to research options associated with the illness.

Transitional treatment and care for the seriously ill.


The Purpose of AIM

 

The AIM program helps patients, who are currently under the care of home health for serious illness, cope and adjust to possible end-of-life care.

What Does the AIM Program Provide?


The AIM program offers palliative care services to assist the patient and their loved ones, should it become necessary, with the progression from curative to comfort care. The AIM Team can transition with the patient to Concerned Care Hospice should they require this pathway of care.


The Goals of the AIM Program:

  • A better plan of care for the patient
  • To provide spiritual and emotional support
  • To provide pre-hospice evaluation
  • Ease of transition from home health to hospice services when appropriate
