Concerned Care’s Psychiatric Home Care offers collaborative, comprehensive psychiatric nursing services to patients in their homes with these and other goals:
For those grappling with mental illness, even home can be a very challenging place. Each person and situation is different. We strive to understand his or her complete circumstances, as well as that of their family and community environments. Our integrated care program is personalized to help each patient flourish in the home setting and beyond.
Registered nurses with advanced psychiatric training, lead our team approach that treats both mental and physical conditions, including patients who are acutely ill with co-occurring mental health diagnoses or who are persistently mentally ill with one or more medical conditions.
Working closely with patients, their families and healthcare providers to coordinate care, we motivate patients to keep their outpatient appointments, and help them find practitioners or other community resources to help them stay healthy at home.
Concerned Care’s mental health nurses provide supportive therapy and monitor medication compliance and effectiveness. And to make sure all health care needs are met, we provide a range of services, including specialty nursing, social work, rehabilitation services and home health aides.
Psychiatric nurses work with your primary care physician or psychiatrist in planning care and also work closely with other care team members or involved community providers.

Services are based on a comprehensive physical, psychological, mental, cognitive, family and environmental assessment of the patient within the context of his/her home. Interventions are provided by licensed, certified, clinical nurse specialists or by nurses with significant experience in psychiatric nursing. * His or her illness manifests by a refusal to leave home—a considerable and taxing effort?
* Or, it would not be safe for him or her to leave home unattended even it there is no physical limitations?
