Hospice is a special
kind of caring. It is a support system of medical, social and
spiritual services designed to provide comfort and support, as well
as preserve dignity and improve the quality of life for individuals
dealing with a life-limiting illness, including family members and
other loved ones.

To be the most compassionate and caring Hospice by helping patients and families accept terminal illness, allowing them to live each day to the fullest and to enjoy their time with family and friends all while dealing with the challenges at the end-of-life.
Concerned Care Hospice accepts referrals from hospitals, physicians, social workers, nurses, case managers, clergy and family members. A hospice patient must have a physician verified terminal illness with a life expectancy of six months or less. A physician’s order is required. The patient must no longer be receiving curative treatment.

Respite Hospice Care:
A benefit defined under Medicare which allows a home hospice patient 5 days of 24 hour care in an inpatient hospice setting allowing a rest period for the caregiver.
